Marriage Certificates
Marriage Certificates are not issued by the parish or archdiocese. Marriage Certificates are only issued by The Government of Ontario.

Baptismal or Confirmation Certificates
Certificates can be made available for you based on request. Fill in the form below with the required information to request for a certificate.
IMPORTANT: Instructions when picking up the requested certificate
The individual authorized to receive the record should come in person to the parish office and present proper photo identification to pick up their certificate. Examples of photo identification include driver’s licence, passport etc.
If the person does not live near the parish and is unable to pick up their certificate from the parish of issue, one of the following three options are to be considered:
- If the certificate is required for admission to a Catholic school, or for the reception of a sacrament at another parish, the certificate can be mailed or emailed directly to the school or the parish where the sacrament is to be administered.
- If the certificate is required for any other purpose, it can be mailed to the individual’s parish of domicile where he or she may pick it up after presenting proof of identity.
- Have a next of kin pick it up by writing a letter of authorization. The letter should include:
- The name of the certificate owner, home address, and email address
- The date of the letter
- The name & address of the person who will pick up the certificate
- Their relationship with the certificate owner (should be the next of kin)
- The reason for the request
- The validity period of the letter
- A copy of the certificate owner’s photo ID
- A photo ID of the authorized person
